Effective January 1, 2008 State Department of Education Changes -
    1. The HEALTH HISTORY Questionnaire MUST now accompany the 'physical evaluation form' when visiting a doctor for a student-athlete's annual athletic exam.
    2. The doctor is now required to sign off that he/she has reviewed the health history questionnaire at the time of the physical. (See Athletic Participation Packet for more information.)

 

FIELD LOCATIONS:
    Football, Field Hockey, JV Boys/Girls Soccer, Track, Baseball & Tennis - fields are located at the school
    Varsity Boys/Girls Soccer - at Green Wave Park in Audubon
    JV Baseball - Mt. Ephraim fields, 3 blocks down on the right from the intersection of kings highway and black horse pike headed towards camden or walt whitman bridge
    Softball - field is located in front of school
    Cross Country – at Pennypacker Park, Haddonfield or West Deptford Park
    Golf - Beckett Country Club, 2387 Kings Highway, Woolwich Twp, 08085
